CARY, NC, June 15, 2015 – Florida Hospital Physician Group (FHPG) and Florida Hospital, members of Adventist Health System, have selected Smartlink Mobile Systems to meet the Centers for Medicare and Medicaid Services (CMS) electronic requirements for Chronic Care Management (CCM) services. Florida Hospital is among the first large hospital systems to participate in the new CMS CCM program, aimed at improving the health and wellness of Medicare beneficiaries with two or more chronic conditions.

Providers who elect to offer CCM services are required to have several electronic capabilities in addition to a certified EHR, including electronic exchange of care plans and clinical summary care records between healthcare providers (faxing will NOT fulfill the requirement), and availability of an asynchronous communication mechanism for beneficiaries to access care management services 24/7.
In addition to being the only vendor that addresses all of these requirements on a single platform, Smartlink Mobile’s auditable event and activity documentation makes it easy to track time invested per patient per month and optimize potential CCM revenue.
